Community Connections: Housing and Support
Requires a
COMMUNITY OUTREACH SUPPORT WORKER
12 Month Full-Time Contract Position: 37.5 hours per Week
Location: Owen Sound

Community Connections is a program of HopeGreyBruce Mental Health and Addictions Services. The program is funded by the South West Local Health Integration Network and Ontario Ministry of Health and Long-Term Care to provide housing and support for adults recovering from serious mental illness.
We are inviting applications for a temporary full-time (37.5 hours per week) Community Outreach Support Worker with the Central Team of Community Connections Housing and Support, which is located in Owen Sound.
This position is primarily responsible for the coordination of rapid re-housing and procurement of items to support successful transitions within housing.  This work includes assisting clients of the Apartment Program to secure and maintain suitable, affordable housing. Primary duties include: conducting income verification and apartment inspections; providing on-going liaison / troubleshooting with tenants, their landlords and other involved mental health and addictions service providers. The position may also include intake assessment; coordinating client placement in rent-subsidized housing throughout Grey-Bruce; outreach community support services including the provision of individual and group supports for independent living (functional assessment, supportive counselling, and case management).
QUALIFICATIONS:
• 3-year Diploma or Degree in applied health or social sciences (i.e. Social Work, Psychology, Nursing) with a focus on recovery-oriented practice
• Registration in a professional college is preferred
• A clear understanding and orientation towards recovery-oriented practice
• Demonstrated knowledge of the principles of property management
• Recent experience in mental health and addictions services within the supportive housing context
• Thorough and practical understanding of mental illness and addictions within supportive housing
• Preference to working with people living with concurrent disorders
• Ability to work independently in a non-structured environment
• Strong belief in a client-directed practice
• Ability to establish and maintain good working relationship with tenants, team members, housing providers and the community at large
• Able to organize, implement and analyze data in order to formulate and carry out an operational plan
• Excellent communication, organizational and relationship skills
• A good sense of humour and the ability to work well under pressure
• Must have a valid driver’s licence and daily use of a personal vehicle
• An up to date Criminal Records Check, including a Vulnerable Sector Screening
