Community Connections: Housing and Support
Requires a
12 Month Full-Time Contract Position: 37.5 hours per Week
Location: Owen Sound

Community Connections is a program of HopeGreyBruce Mental Health and Addictions Services. The program is funded by the South West Local Health Integration Network and Ontario Ministry of Health and Long-Term Care to provide housing and support for adults recovering from serious mental illness.
We are inviting applications for a temporary full-time (37.5 hours per week) Community Outreach Support Worker with the Central Team of Community Connections Housing and Support, which is located in Owen Sound.
Position duties include assessment and treatment planning, supportive counselling, crisis intervention, case management and one-to-one and group supports for independent living. The position involves mild to moderate physical activity, including the provision of practical assistance to clients in household management and maintenance tasks. The position may require some evening and weekend work. The position is now available.
• 3-year Diploma or Degree in applied health or social sciences (i.e. Social Work, Psychology, Nursing) with a focus on recovery-oriented practice psychosocial interventions
• Registration in a professional college is preferred and may become a requirement
• At least three years of recent experience in mental health and addictions services 
• Thorough and practical understanding of mental illness and addictions
• Demonstrated ability to promote and support individual recovery
• Experience related to concurrent disorders would be an asset
• Familiarity with psychotropic medications and experience in assisting clients to manage their medications and their physical health needs
• Demonstrated skill in assessment, psychosocial rehabilitation, supportive counselling
• Training and skill regarding motivational interviewing and crisis intervention
• Excellent communication, organizational and relationship skills
• Strong team skills and ability to work well with community and professional groups
• A good sense of humour and the ability to work well under pressure
• Proficiency in use of personal computers for email, data entry and word processing
• Must have a valid driver’s licence and daily use of a personal vehicle
• An up to date Criminal Records Check, including a Vulnerable Sector Screening

Nurse Practitioner

The Brockton and Area Family Health Team Is inviting applications for a  Nurse Practitioner – Primary Care Telemedicine Program
Permanent Full-time Position, 37.5 hours per week

In partnership with HopeGreyBruce Mental Health and Addictions Services, we are seeking an enthusiastic and innovative Nurse Practitioner to deliver the Primary Care Telemedicine Program.  This program supports improved access to primary care for adults with mental health and/or addiction problems. This position is based at our Walkerton site, with regular travel to the HopeGreyBruce Owen Sound office and other satellite sites in Grey Bruce required.

Position Summary
• Provide a full range of primary health care services within the NP scope of practice, including assessment, diagnosis, treatment, follow-up, education, consultation and referral
• Provide primary care services to people living with a mental illness and/or addiction who are receiving services within the mental health and addiction system; services will be delivered in collaboration with local mental health, addiction and primary health care services
• Work in partnership with a RN or RPN who will provide direct and indirect clinical services and will serve as the Clinical Telemedicine Coordinator for the program
• The services of the NP will be primarily offered via the Ontario Telemedicine Network


Position Qualifications
• Baccalaureate degree in Nursing
• Valid certificate of registration in the Extended Class from the College of Nurses of Ontario, and eligible to practice as a RN-EC
• Experience in a Community Health or Primary Care setting
• Demonstrated ability to communicate effectively with patients and members of an interdisciplinary medical team
• Strong interpersonal, leadership, problem solving and conflict resolution skills
• CNA certification in Psychiatric/Mental Health would be an asset
• Clinical experience in the mental health/addictions field would be an asset
• Experience with OTN and telemedicine would be an asset
• Proficiency in the use of computers and information technology
• Valid drivers license and access to a reliable vehicle
